Hey people, I had a question regarding this. I know it occurs in people who have had diabetes, and eventually all they can see is light from dark. I think this is what happened to my dad recently. He just told us this weekend that for the past 2 weeks he has lost almost all vision in his eye. He finally went to the Dr, and has an appt. with the specialist tomorrow. I was wondering if anyone knows what percentage of people can get most of their vision back, or what the future outcome may be since this happened two weeks ago. I am worried that perhaps the time that has gone by has reduced the possibility of getting some sort of resolution. It has been confirmed on Thursday that he does have a leaking vessel in his eye. I thought I would ask to get additional info. Maybe their is another cause? Thanks!🙁

Has he been seen on regular yearly basis by an OD or OMD? Does he get a yearly physical with a PCP with bloodwork? I don't believe if I saw a leaking vessel in an eye anywhere near the macula would I sit on the patient FOUR days. Is his vision 20/20, Count Fingers, Light Perception? High Blood pressure out of control, diabetes screened before, african american for sickle cell trait?

A lot of vascular problems could be differentials: BRVO, CRVO, CRAO, Diabetes, Hypertensive Retinopathy, Macular dettachment, Retinal dettachment (low on Differential if there really was a leaky vessel), etc.

Thanks! Unfortunately, my dad will not assist to the DR, unless he is/has been sick for a long......while. Thats why he waited 2 weeks before telling anyone he couldn't see all of a sudden. His vision otherwise is perfect, being that he is 60. He does get a physical through his work annually, but no bloodwork I think. I can't recall the last time he actually went to get his vision checked, unless they do that during his routine physical now too. BP is within normal range. My biggest concern is his diabetes which he has had for about 10years that he only tries to regulate when its time for his physical, for work. The last time he checked blood sugar it was something like 304 he said. (last week)Not so good. 🙁 Well, I guess I will know more info by tomorrow after his appointment!

Highest Differentials: Diabetic Retinopathy (non vs proliferative), Clinically Significant Diabetic Macular Edema, All good topics to read up on.

Make sure he keeps all his eye visits. You might research some about laser photocoagulation.

There are many differentials in this case. The best thing for you to do is accompany him to his appointments and ask as many questions as possible. If he has a vitreal hemorrhage he may regain some of his vision or all of it, but it takes a while (sometimes up to 3 or 4 months) for this condition to resolve. He must also control the systemic problem that is causing his vision loss. If it is not controlled, he may ultimately have more problems than his vision.

Sorry to be so blunt, but you need to know this so you can educate your father.

Thanks, I can't make it to his appt today. (crazy boss wouldnt let me) But my mom is. I will find out what is going on and whatever it is, like you said needs to be controlled. I'll update when I know more, and I will read up on those few things.👍
 
There is a paucity of clinical data and actual history but what is likely to give Hand motion vision or LP vision is a large vitreous hemorrhage in the presence of PDR. CSME could knock central vision but usually doesn't give the feeling for a patient of "losing their vision". In addition, the sudden nature of this doesn't lend itself to CSME no matter how large it was. BRVO's are not usually profound enough, either, to cause a demonstrable loss of vision.

If the patient is hypertensive of questionable compliance or control, then a vascular event posterior to the optic nerve head is also likely.
