So, I am very unhappy with my current program and would like to switch in to another program at my hospital. With whom would it be appropriate to start this conversation about switching in to another program at my facility, do I start with my PD, the PD of the other specialty, or the ADME?

From a tactical point of view, you never want to upset your program or alert the ACGME until you are really really sure you are going to make the change. Best to talk to the other PD first and get his thoughts. He may dissuade you or tell you it's unrealistic. I know a few people with lackluster CVs who were in noncompetitive residencies and out of the blue decided the derm or rad onc lifestyle would suit them better. After a sit down with a PD they had a better sense of reality.
