As a third and fourth year medical student...is it protocol to call the residents "Dr. So and So" or by their first name?

Most students call residents and interns by their first names. The only service that I was on where the residents actually asked to be called doctors was an OB/Gyn rotation at a community hospital. They were all very weird.
 
why not just introduce yourself to them first? and see how they reply?

eg Hi, I'm John smith, 3rd year medical student (then handshake/greeting/secret society handshake!!)

"*grunt* I'm (DR!!!) Thompson"


then you know... Dr Thompson it will be.. and not pompous git as you KNOW you should be calling them
 
Pay careful attention when they introduce themselves. Most will use their first names, the ones that don't, call them what they want to be called because it's not worth the ruckus and they're the ones it's important to. If in doubt, or if you forget, use "Dr. So-and-so" and if first names are okay, they'll correct you.
