There is a section that under PTCAS that asks if I was involved in "unacceptable academic performance or conduct violations"?

Does this only pertain to academic misconduct?

I was on disciplinary probation for a month when I was caught underage drinking on campus at UC Irvine. However, I am not sure if that would apply or not. Help please! 😕

i believe by conduct violations they mean academic dishonesty. Also, I hope you aren't trying to complete some sections of your ptcas app right now because apps for the next cycle won't open until July. So anything you put in now will be deleted.
