Has anyone done a rotation in Puerto Rico? I'm trying to polish up my spanish which is already fairly good and was thinking Puerto rico would be a nice place to do that.

Haven't done a rotation in Puerto Rico myself, but you might want to look into Ponce Medical School to see if they accept visiting students.

Ponce SOM is the most well known/respected school in PR with faculty from some prestigious institutions in the US (they will speak English to you if needed), but, it will probably depend on what rotation you want on how much the school may accommodate you.
