Q about prelim year

It's possible, but you must find an open PGY-II slot, which are certainly available, but harder to come by.
 
I'd say it straddles the border between 'common' and 'scarce.'

Seriously though. It's not really that uncommon to do this. The problem is that most residencies have only a few slots per year. You have to wait for a PGY-II slot to open, then apply there with lots of other residents who also want to start as a II or change programs. Look at the psychiatry clearinghouse (do a search here for the link) and you'll see what's available.

University of Nevada - Las Vegas has all its residents begin as PGY2s.
 
at hopkins, they have I believe 2 spots for pgy-2s who sign up before internship year outside of the match. these people do an internship at the place of their choosing and then join the residency as a pgy-2. basically the program gives you the option of applying either as a pgy1 or as prelim plus pgy2 but i believe they then make agreements outside of the match for those who choose to go the prelim route thus they avoid having to deal with the match.

also, penn has 1 spot every year for pgy-2's to come into the residency after a year doing an internship. often this has been reserved for people switching from other residencies into psychiatry like medicine or surgery.

i came across a program in nyc that offers a few slots to pgy2s ... i'd have to look it up again, but i believe it was St Luke's/Roosevelt in manhattan. dont know much about the program though, i just remember reading that, because i might be doing an osteopathic traditional internship, and i hate the idea that im prolonging residency by a year ... i have no idea how the application process goes for that.
