If I am accepted to an early assurance program and end up doing very well on MCAT, I may end up deciding Id like to apply more widely to higher ranked schools. I would then have to decline my EAP admission. My question is will I have to declare that I have previously applied and been accepted to a med school and if so, will this be held against me in later cycles?. Thank in advance.
Nope. If you have not submitted an application via AMCAS, you have neither applied nor been accepted to a MD school. Your EAP acceptance is contingent, isn't it? If so, one of the contingencies is not applying elsewhere, isn't it? 😎

If so, you haven't officially applied, and you haven't been "accepted." Nothing to declare. What you are contemplating is done every year, by a ton of people spread across many schools across the country. They are not "reapplicants" who have turned down a prior acceptance. The contingent acceptance you would be turning down would be for the same cycle in which you are applying!
