Question about AADSAS Matching form

On the AADSAS matching form, as far as the months attended go, is accuracy here really important? In the past I've attended several other schools in addition to the college I got my degree from, but I'm having a really hard time finding out the exact month I started and finished at some of these schools. So is it ok for me to give my best estimate of the month (with the year being correct), or will they make me redo any forms that have a date slightly inaccurate?

Also, there was a community college I went to each summer, so do I need to do multiple matching forms for each consecutive term, or just one matching form for all the times I went?

each college should have a separate matching form. Basically, if you are submitting a transcript from somewhere you'll need a matching form. I'm assuming you have one transcript from the community college that will include all the summer classes you took over the years - thus you just need one transcript matching form.
