Do the courses need to be finished before application or before matriculation?

And are General Bio, General Chem, and Orgo the main preparation one needs for the MCAT (in terms of Bio and Chem, not Physics and Math) or is one at a disadvantage if you don't have upper division bio classes?

There are definitely threads about this already on here. The question is - it depends on the school. A few require everything before you apply. Most require everything to be done before matriculation (beginning medical school). And many have months (UC Davis, June before you start, UCSD, August 1st is their last day to have them done before you start). But the best source is either the MSAR or the website of the schools you're applying to. I'd get a whole year of physics and gen chem out of the way. I'd take the first two quarters of ochem then wait on the third because many (and starting to be more) don't require the whole year (go figure...), but biochem is also starting to be required (but figure out where you're applying first). K... that's all...
