Question about some IM residency locations

Hi on the NBME document with the IM board passing rates, it gives separate numbers for ie, Mt sinai - Elmhurst, Mt sinai - NY, etc, so they separate it based on the location. So i'm wondering if I apply to Mt sinai IM residency, am i applying to all the locations at once, or do I have to submit one for each? And when people say Mt sinai, do they automatically mean the NY one, and elmhurst is not counted?
 
They are not as a competitive. Same thing with UCSF and UCSF-Fresno, or Johns Hopkins and Johns Hopkins-Bayview.
 
there is the main university program and then there are university-affiliated community programs. when someone uses the name of the university program they are talking about the main university program and that is the prestigious program

also this isn't unique to IM
