For all of the practicing pathologist out there, what was the most difficult issue you had to deal with when first starting out? Also, what have you changed regarding your grossing and reporting in comparison to residency? Do you find yourself being less anal or more anal?

The answers to your questions are quite variable depending on what type of practice you enter and who you are working with.
 
Regardless of what kind of pathologist you are, it's a bit scary at first for most of us the first time we sign out a case ourselves. A good first job will have colleagues you can discuss challenging cases with, but even the routine ones get you worked up at first. Finding the balance is key, and remember that even experienced pathologists show cases. I'm in FP and it's true here, as in diagnostic path.
