Question about supplemental fees

Does someone have a copy of the 2008 AADSAS pdf with the supplemental fee information for all the schools or a link to the 2009 version? AADSAS seems to have taken the 2008 version off their website. Thanks.

What else are we supposed to send besides the fee? Is there an AADSAS matching form or something? And what if we are taking the DAT later this summer, should we hold off and send our DAT and fee together?

There are no AADSAS matching forms for the supplemental fees. I just attached a note specifying my AADSAS ID, address, etc.

I would also send your supplementals now even if you are to take the DAT later. For some schools all it really does is activates your file.
