As I was filling out my application to the state board of pharmacy today, the question "Have you ever been diagnosed with chemical dependency?" was prompted.
Truth is, I went to rehabilitation when I was 16. I am now 21, and have overcome the struggle of alcohol abuse/addiction and have disassociated myself from any involvement with it at all.
Question is, being that I was a minor when I went to rehab, can I choose no to dispense this information? I feel like it would greatly hinder my possibilities of being approved, and I've worked so hard at the Tech School to get this far. Please give me your honest thoughts.
