Questions about California intern hours form

Hello,

I don't understand how to fill out California pharmacy intern hour affidavit form. On intern hour affidavit form, it has this:

A total of 1,500 intern hours is required but does not have to be obtained in one pharmacy location. Please
indicate below the number of hours the intern
pharmacist
obtained while under your supervision.

I have 3 questions

(1) I understand hours don't have to be gotten from 1 location. so for the 1st line to fill out, so if I worked 100 hours at 1 site, my preceptor would put 100 on 1st line, right?

(2) On 2nd line, what are they asking? What do they mean with the 600? My understanding is that CA accepts 600 hours from school rotational hours and 900 have to be gotten on your own outside school in a pharmacy, is this right?

(3) If I finish 100 hours at a site and know I'm done with that site, do I just mail this form when I'm done with that experience to CA board or after I graduate with whole application?

From my understanding work experience and rotations both would go on the first line because you were actually practicing within a pharmacy. Your school can give you up to 600 hours which would be the second line.

Keep the forms with you until you graduate. I don't know how all of the CA schools do it, but at least a few of them will collect all the application materials from the entire class and mail them to the board in one large package.
 
The first line is if you worked in a pharmacy or hospital pharmacy or doing clinical stuff at hospitals.
The second line is if you had an "academic" rotation where you didn't work in a pharmacy, but your rotation was related to pharmacy (still has to be with a PharmD preceptor, not PhD). Another example is if you had like a "management" rotation where you were with a district manager or something.
Majority of your hours should go on the first line.

I had about 1200 hours on the first line and and 400 hours on second line. CA accepted all 1600 of my hours from my school rotations...
