questions about the match

I have a couple of questions that I'm confused about...

Since I'm applying to both categorical and advanced PM&R spots (start at PGY2), how does the ranking list go? Is there a separate list for TY with advanced PM&R programs?

How many transitional year programs do most people apply to? If I don't match into one, are there usually spots open for which I can scramble into? I've only applied to 4 TY programs.

I'm a D.O. and I've elected NOT to apply for the AOA intern year based on the fact that if I match, I would no longer be a candidate for the categorical PM&R programs since the DO match is earlier. By doing this though, I essentially limit my chances of also using the AOA intern year for PM&R programs that accept it as a prelim year. Am I making the right decision here?
 
You rank your TYs/prelim years secondarily to your categorical list.

i.e. rank your categorical programs in order, then rank your TYs in order for your first rank, then for your second and so forth. This way you can do a separate TY year list for each categorical spot you rank. (i.e. if categorical rank #1 is in chicago and #2 is L.A., you can rank a TY list specific to matching at the chicago program. You also will rank a TY list specific for L.A., so if you match at LA, your secondary list for LA will be used to match you with a first year spot...the chicago TY rank list will thus not be used.)
