I am greatly interested in going into plastic/reconstructive surgery. Right now, I will be starting a post bac program to enhance my medical school application. I was wondering what I need to do during my post bac and my first couple of years of medical school to be very competitive in applying for these residencies. I know grades and STEP scores are important, but what else do I need to look into doing? (Research, shadowing, etc?)

Get published... The closer to first/second author, the better.
(Good journals, especially PRS ones).

How would you even get in on that kind of research? I imagine the process for publishing takes a long time (more than a few years).

Once you're in med school, get to know the plastics people and do research with them.... Or find a program that you can take a year off doing research with. Not to worry now though...get to med school first.
