MD Questions regarding 4th year grades

Hi everyone, was wondering if anyone had some insight into some questions I had regarding fourth year grades.
I go to a school that reports about 3-4 fourth year electives (Sub-I, random clerkships, etc) on our MSPE. For context I will be applying IM.

I want to preface by saying I understand this question is probably quite neurotic, and understand that I am not in a bad position.
If this causes any eyerolls, honestly I completely understand, but I promise you I genuinely do not know the answer.

I go to a T20 school and received full honors during MS3.
My program does not freely hand out honors so typically 1-3 students a year honor everything.
As a result, my dream is to apply to top IM programs.

1) If I were to not honor my fourth year Sub-I, would this be seen as a concern to program directors given the context of my better performance during MS3? I guess my concern is that a lower grade would be seen as a "red flag" for me since it might look like I "regressed" in a way.
2) I also have two other planned 4th year clerkships in IM subspecialties. Would additional honors help my application given they are IM related electives, or does it not really matter?
