I'm just applying for the M.D. program.......do I need to even worry about pages 5 and 6 of the application, since I'm not doing the MD/MPH program?

Would I just mail in the first four pages of the application when I send it in?

yes, if you look at the application instructions, there is a section called "submission checklist", which states that the first thing on the list is the "supplemental application form: pages 1-4". It IS confusing though, cause they ask for the MCAT score in that section, but I'm just sending in pages 1-4 like they ask for.
