would one month of volunteering in an eye clinic be worth mentioning on the application? or is it too insignificant?
it certainly wouldnt hurt. that would be something that you could mention and use as a reference to any knowledge you may have learned, but its not something to fully stress because of the length of time.

That depends on what your duties were at the clinic. Did you just answer the phones/put files away?
Or did you get hands on experience helping out the doctor, interacting with patients, conducting pre/post tests?
 
Yea, I'd add it, I'd also add how many hours you worked (which is way more informative than the dates worked) and what your duties were, as well as your reference.

No detail is too insignificant. If you ate a sandwich, tell them what kind it was, what type of bread, date eaten, place, your rating of it, and its nutritional content. People love information.
