See below. For the OP, not to worry about useless responses. They don't see the light of day on this forum. 😉

My own suggestion would be to have a talk with a medical licensing specialist lawyer in the state you think you might practice. There are usually a small group of experts in this issue and they could likely answer the questions related to how their clients have dealt with hospital related issues.

My experience in seeing folks with a lot more issues and more recent ones than you obtain hospital privileges is that it is unlikely that hospitals will refuse to credential you if the licensing board was okay with it. However, I am not an expert on this.

I have a history of several ( >5, < 10) misdemeanor arrest all involving alcohol. They took place durring a five year period durring highschool and college. It's been 8 years since I've had any trouble.

I've been upfront with the medical licensing board and my program director and I am in recovery. It will have been over 12 years by the time I am finished with training. If anyone has any information on what hurdles I will face when looking for work, getting hospital privilages an insurence I would appreciate it. Please no, "wow >5", comments. I've been through a lot and have overcome a rough start in life. I don't need grief from anyone, I've had enough.

The correct answer is: discuss with a lawyer.

That being said, I think the long documented history of a lack of any further problem will outweigh your history. There is always the chance that some state board will refuse you a license, or attach restrictions like ongoing monitoring, and you can be certain that your licensing will take longer than usual.
 
The biggest hurdle will be getting a license. Insurance and privileges will come after that, but are easier to obtain than the license.

ALWAYS be proactive in contacting the Physician Health Program in whatever state in which you intend to practice earlier than applying for the license. Don't wait for the state to ask you. You will be required to make a personal appearance before the board - that's a guarantee.

Be advised that, if you do relapse, plan for 90 - ninety - days, not 28, in treatment.

And, as recently told to me by a guy "in the know": as it is now, if you do not have a license in California, and you have any drug/alcohol history, pass CA by for now. Their PHP imploded, and new systems are not in effect yet, and may be a while.

I have some experience with this. I am currently on the road to med-school and have done some research on this topic. That being said, I'm not a lawyer and this should not be taken as legal advise.

I have three misdemeanors on my record from ten years ago when I was fourteen. I, like you, was concerned about these. So here are the things that will help.

1. Talk to a lawyer to see if you can get things taken off your record. You may be surprised.

2. Be honest. Even though my arrests have been taken off my record, I still need to admit them to the licensing board. Lying will get you in more trouble than telling the truth every time. Just look at the minutes from the last board meeting. There will be multiple people who lost their license or did not get accepted because they lied.

3. The state examines each case on a case by case basis. So just because one person got denied or accepted does not mean it will happen to you.

4. Main things that they take into account are the time since the crime(s), severity, youthful infractions, and how relevant it is to the profession.

5. Here is a list of crimes that one would have a harder time getting licensed/hired with;

Assault (1st, 2nd, 3rd degree) Criminal Mistreatment (1st and 2nd)
Assault (custodial) Custodial Interference ( 1st and 2nd)
Assault (simple) Extortion (1st, 2nd, 3rd*)
Burglary Forgery*
Child Abandonment Indecent Exposure-Felony
Child Abuse or Neglect Indecent Liberties
Child Buying and Selling Kidnapping (1st and 2nd)
Child Molestation Malicious Harassment
Communication With a Minor Manslaughter
Criminal Abandonment Murder (any degree or type)
Patronizing a Juvenile Prostitute Promoting Pornography(1st)
Promoting Prostitution (1st) Prostitution
Robbery (1st, 2nd) Rape (1st, 2nd, 3rd)
Rape of a child (1st, 2nd, 3rd) Selling/Distributing Erotic Material to a Minor Sexual Exploitation of a Minor
Sexual Misconduct w/Minor (1st, 2nd)
Theft (1st, 2nd, 3rd*) Unlawful Imprisonment
Vehicular Homicide Violation of a Child Abuse Restraining Order

I hope that this helps. Many people do stupid things when they are young and later regret. I know that I am a much different person than I was when I was fourteen.
