How many questions on the step 1 require you to identify radiological images or histological images? How many audio questions (where I assume you listen to heart beat abnormalities)?

I dont think there is a set number.
My friends exam, who took it on the same day as me in the same center, had 2-3 audio questions in each block! So total of around 15-20... I on the other hand had 3 audio questions total only...
Also, there can be a varied type of audio/media questions. Heart sounds, gaits, reflexes and respiratory sounds can be present. I had two heart and 1 respiratory if i can remember clearly... Some random person in the test center had a video of intention tremor lol i noticed it as i walked out to take a break.

Any advice on how to prepare for these questions, especially audio ones?

The media questions are generally easy, really...
You can pick out in what area the murmur is straight away, then you just have to figure out what kind of murmur... What i did is figure out if its diastolic or systolic.. For diastolic it is ARMS PITS... For systolic its everything else... If u know the area n this info you can id the murmur most of the time.

Ofcource there are things like PDA n MVP that occur with a distinct sound which help you id the murmur.
Youtube has tons of examples if you really are scared about this type of question.
