Hoping someone can help answer these questions.

1) Major cause of M/M in those with Parkinsons:
- Postural Insatbility --> Falls
OR
-Impaired Swallowing Reflex --> Aspiration

2) We all have learned that to Dx. PE, V/Q mismatch is typically the answer UNLESS they ask for the "most diagnostic"...which is then CTPA.
- I came across a newly edited question in USMLE World, the answer was CT Pulmonary Arteriogram (for those with OK renal function)
- so...which is it? V/Q or CTPA?

3) When to order "Fasting Lipid Profile" vs. "Total Cholesterol Screen"
- I always get this wrong!!

4) Back to my HT in pregnancy question: which one?
- Labetalol?
-Methyldopa?
-Hydralazine?

5) Viagra and Nitrates...
- how much time has to elapse before taking the viagra?

2. well V/Q mismatch isn't always the best. for example if you have a patient who had a partial lung resection for whatever purpose, you'll have a V/Q mismatch in which case you need to do a CT. UW doesn't really make it easy for you to know which one to do especially because it feels like they give you all the possible choices and then the correct answer is the one you wouldn't think of
3. i don't even remember coming across that question but i remember that various sources conflict with that. Some recommend starting at age 20 with 5 year fasting lipid screens but if you want the answer from USPSTF, they recommend 35 for men and 45 for women (which is hopefully CK mirrors)
