rank order list dilemma

i have a problem. i'm sure there are others who would like to know the answers to this dilemma. i plan on ranking both 3-year and 4-year anesthesia programs. by 4-year, i'm referring to those programs that integrate the pgy-1/preliminary year into the categorical anesthesia residency. from word of mouth, i heard anesthesia applicants must submit 2 different rank order lists: one for anesthesia, the other for preliminary year programs. now suppose in my anesthesia rank order list, i rank both 3-year and 4-year anesthesia programs. and then i rank separately my preliminary year programs. what if i match into a preliminary year program AND a 4-year anesthesia program?? i'm so confused and i'm sure i'm not the only out there...help!!!
 
Correct me if I'm wrong but if I remember correctly, but if you match to a categorical program, your preliminary list will become inactive. You will not be held to a commitment to a prelim year when you already have a categorical clinical base year in place.

BTW, for those of you who intend to just scramble for a prelim/transitional year and save money and time on the interview trail, I've heard some stories recently from a couple of incoming CA-1's for our program that would make your stomach crawl. Give yourself some options if possible.
 
So, if I understand correctly you're saying that if your match is to a categorical program (i.e. a 4 year program in which the 1st year is your intern year) then that automatically drops you from any separate matches to other transitional/prelim programs?

On the other hand if you match to a 3 year program (no prelim year included) then you have to find a prelim/intern year--so it's better to enter the match for a prelim year in case you match at a 3 year residency program so you don't have to scramble after the match?

I think that's what you're saying, I just wanted to verify it. There is a lot of confusion involved with this whole match thing.

You just have to rank 3 and 4 year programs in the order that you want them. For the 3 year programs, there's a supplemental list where you can separately rank 1st year programs any way you like. If you apply to a program that has both, you can rank them consecutively on your rank list

you are scaring me with these stories that "would make your stomach crawl". i was planning on trying to scramble for a pyg-1 spot because the program i want to match at for anesthesia only has advanced positions and i only got one interview as far as ty programs are concerned and didn't apply to prelim medicine programs. it was my presumuption that scrambling into a prelim spot somewhere in the country in not a big deal--that there are plenty of spots that go unfilled. can you elaborate on your stories please.
 
There are plenty of spots that will be available but most will be in some pretty ****ty locales and/or ****ty programs that will beat you to death. One of our incoming CA-1's has been e-mailing me from Mississipi and she has been just incredibly miserable and overworked with some creative time keeping to keep her under the 80 hour work week. About 70% of my med school classmates that did that also had pretty to extremely bad experiences.
