ranking prelim programs

does the reputation of your prelim program matter at all in the long run, i.e. for potential fellowship apps? better (and typically harder) programs will probably have better teaching, etc but is it worth the blood, sweat & tears if we're ultimately practicing dermatology??
 
Presumably, you'll only end up at a ticky-tack internship if you match into derm. If you're going to rank a prelim program as a backup, it'd be stronger and at the end of you primary list, not in a supplemental list.
 
but for prelims in the supplemental list, would it be worthwhile to rank a good academic medicine program higher than "cushier" community programs? do fellowships consider where you did your prelim?
 
I would think that fellowships would care more about where you did your derm residency than your prelim...many people who are even at the top derm programs do prelims at cush places...i don't think it matters...
