Reapplicant Transcript Matching form

I cant seem to find the transcript matching form. I clicked at my school ,but when I hit print, it doesn look like the form I submitted last year. Its just shows printing a copy of what my screen looks like.

Having the same problem, also reapplicant. It says there should be a symbol that doesn't exist. Also won't let me add in this last spring semester, appears to be "locked"
