I am planning for d-school the next year and so i will be applying this may. Now I actually got one LOR from orgo professor which was last year. During that time I didnt knew anything regarding LOR that u have to attach some form/paper from ADEA website which should be attached with ur LOR, so I dont have it. So what do u think I should do now? can I just go and tell my health advisor to attach that ADEA form with my LOR?

Put the sealed letter of recommendation and the cover letter into a another envelope. 🙂
