I am applying for an MD/PhD and am gathering my recommendation letters. Currently I have three solid ones (one from my biochemistry PI of 3 years, one from my psychology PI of 1 year + professor for 4 semesters, and one from my biology professor/mentor since freshmen year).

I am ansty about the fourth. I was told NOT having a recommendation from a summer research program is a red flag on applications as it suggests I did not leave a good impression.
But I am reluctant to ask a recommendation letter from a PI I barely saw over the summer who left me to work fairly independently in a separate lab during the entire summer program.
I would much rather have my fourth be my second biochemistry PI/professor. He can attest to my grades, my work ethic since I am his TA, as well as my usefulness in research since I juggle his research simultaneously with my first PI.

Would choosing my second biochemistry professor over my summer PI detract from my MD/PhD application?
 
You are correct in that a rec from someone whom you had minimal contact will do you no favour. The exception is HMS which requires recs from all PIs you've worked with.
