If my uGPA was not good and almost all of undergrad with less than stellar grades around ~2.8 (but post-bacc + grad GPA ~3.9) due to not having a certain path for medicine/being unmotivated/bad study habits, do you think I need to talk about my grades in the secondary box asking for extenuating circumstances or anything concerning for adcoms?

I recommend answering the prompt just as you have written above. Not everyone comes to Medicine via an evolutionary path. For some, like you, it's revolutionary.
