When we apply, if we presented at any research conferences that were relatively "open" will we need to make a special note of how "open" they were? I understand some research conferences are selective and some are less selective.

Also how impactful is presenting at regional conferences? Is it right in between local conferences and national conferences?

No. Presenting your research at any professional conference is a positive mark on your application. I'm sick and tired of pre-meds using conferences as a check mark on your app. The point of a conference is to 1) get your ideas out there and get feedback and 2) meet other scientists in your field and network. If you're at a basic science conference and want to go into medicine, you probably won't get a lot out of networking but the main point is still to get your research and ideas out there. That should be your intention - worry about the med school application stuff later. If you're at a medical conference, this is a good opportunity to meet people - no telling what connections will help you out in the future.

I mean the phenomenon of opportunity cost does exist, so gauging the relative value of things shoud never be a bad thing. Thanks for the feedback.
