Appreciate the guidance of any PD/faculty plus thoughts from everyone:
So I got 3 clinical letters which have all been uploaded to ERAS. I also asked my research mentor for a letter and checked that it should be sent to the programs I applied to. Despite months of polite nagging, I don't know when/if this 4th letter will ever come in.

So is it better to just leave it be, and let programs see that I had a letter that never came in? Or do I go into ERAS uncheck this letter? What do I do? Thanks

Honestly, I don't know that I have access to find out if a fourth was to be uploaded and never made it. If I do, I have never really noticed it. I just see the ones you successfully submitted. Probably not a big deal, but probably wouldn't hurt to uncheck it if you think it is never coming.
If it is that hard of a letter to get, it is probably not worth having because they obviously put no great importance on getting it done. They probably would not put much into writing a good one. I would cut your losses and stick with the 3 you have, which is plenty.
