I have heard of stories of navy GMO's who once completed there 4 year GMO training that they have to repeat internship year to enter residency I currently would like to pursue an anesthesia residency If I take a navy transition year would I still have to repeat my internship year? Anyone in or out of similiar circumstances??

Did 2 tours as a flight surgeon (Navy). Civilian anesthesia program went right to CA-1. I would have thrown the bullsh t flag had they made me repeat.

You might have to repeat a portion of you internship year if you do a TY internship but then decide to do gen surg, IM, etc.

Some other things to think about...

Lets say that you do a military internship, and the navy decides to call what you did a Family practice internship because it was at one of the FP residency programs the Navy has.

Now, your locked onto only 2 more years of HCFA funding for training at a full 1.0 rate, and if a program takes you into their CA-1 spot, then they will get 2 years of 1.0 and the remaining year will be reimbursed at a 0.5 rate.

now HCFA funding may not be the bulk of a training programs funding, but in some programs, it may make you less competitive.

now, lets say you did a Transitional year, and then go do anesthesia you will have all three years at the 1.0 rate.

So, yes, what internship you do matters.

Now, as far as repeating it, I know some people do, but I don't plan on repeating internship.

One could even make the argument that after 3-4 years as a GMO, you should repeat it just to relearn hospital medicine, but I don't hate myself enough to go through internship again.

i want out

does it mean that you served in the navy 4 1/2 yrs post internship(1/2yr of fs training + 2 2yr tours)?

Anon: Yes

i want out: You raise some very good points to consider. However, in my experience 1.5 years ago, none of these concerns were ever raised to me during the application and interview process (anesthesiology). The Navy decided to short notice me to Afghanistan during interview season, which brought it's own headaches.
I was surprised how fast inpatient care came back to me.

A very true and accurate statement. This is just one of the many serious problems associated with being a GMO/FS/DMO. One's ability to practice medicine in the inpatient, especially critical care, setting becomes greatly diminished and atrophied as a GMO. Not at all different really than attempting to practice inpatient medicine again after having used hospitalists to provide that service for a lengthy period of time.

For this very reason, I requested to spend additional time at the PGY-1 level working strictly in the inpatient setting prior to advancement to PGY-2, even though I had completed a full intern year prior to the GMO tour. I strongly advise all others do the same. GMO's/FS's/DMO's who think that they are fully prepared to walk right back into the inpatient setting at the PGY-2 level and operate safely and successfully are delusional.

Delusional? That is a pretty strong statement to make. While I agree that for certain specialties maybe some "resitern" rotations to get caught back up would be good.......repeating an internship is just not necessary.....unless you change specialties and have to repeat.
For me, I did ALOT of moonlighting in civilian ER's to keep sharp and make some extra money. Anesthesiology is also a specialty that holds your hand when you start your CA-1 year, so it pretty much didnt matter if you were a brand new intern graduate, or been out for awhile.

Just some observations that I have made.

IMHO......nothing magical about inpatient medicine. At least not enough to repeat internship.

I agree however, critical care medicine is a whole nuther ballgame.

ER not inpatient? Not where I worked. I was responsible for the house at night (rural hospital)
