Request for DAT report

How can I ask ADA to sent additional DAT reports to the 3 new schools I just added on my application. I tried from here: here but it seems like there is some problem with their java script. I cant print out a request form nor the lick to the online request is working for me. Could you guys try it too and let me know if it is just me or they are having a problem.here

I also have a question and would like to get an answer. I can I check and see which schools I requested to get my DAT score sent to? I forgot which schools I put on the list and which I left off.
