Requesting DAT scores

Can we go ahead and request DAT scores to be sent to schools?
In my case, I took the DAT last year and want to send those scores to schools
again this year. I went to the website:

but I can't find a form with the list of dental schools. I thought there would be a form
so I could select the schools which I want to receive my test scores.

Here's the form...but for the list of schools...I don't know where you can find that...I'd assume that you just write in which schools you want your DAT scores to be sent to on this form!
