Residency far from where I want to practice?

Hi all, I hope you guys can give me some input on this.

How beneficial is it to do residency in the city/area one would like to practice long term? Would it be a serious disadvantage if I did residency on the east coast when I want to be on the west coast eventually? What about fellowships? How do they come into play?

Just a little background, I am going to be an MSI next year. I have some conflicts of interest as to where I want to attend med school and even residency. My family is in CA and I would like to practice there. However, my boyfriend and I would like to spend some years in NYC before we have parental duties. If for example, I did my residency in NY, how practical is that if I wanted to practice in CA?
