So I've read around here on SDN that in order to have a good shot at IM fellowship, you need to match into at least a mid-tier residency program. Is there a hard cutoff of what's considered upper/mid/low? It's not hard to find residency program rankings but is upper tier like top 5-10? Top 20? I'm matriculating at UW via the WWAMI program so my in house program would obviously be near the top of my residency program list. According to Doximity, UW's IM program is #11 for reference.

There is no hard cutoff man. You know this. Just do well in school and match at the program that’s the best fit for you and your career aspirations. As you go through school, you’ll learn what’s what.

What fellowship are you interested in? For even the most competitive specialty, GI, an IM program in the top 50 or so will give you a reasonable shot assuming you do your part, top 30 a very good shot. Worry about med school now and Step 1 eventually later.

Right now I’m interested in Cardiology and Pulm/CC but I know I’m way far off from worrying about that.

Just do well in school and your USMLE and go to the best residency you can. Getting into cards fellowship somewhere isn't too hard, being an AMG gives you a leg up already. There's ~900 fellowship spots, it nearly impossible that only those who go top 10-20 residency can get a spot.
