Should i have to give USMLE ?
Should i have to complete General Surgery PG?

By your grammar, I'm assuming you're a foreign medical student. First off, let me say that it is harder for IMGs to get neurosurgery spots. It IS possible, but harder.

How do you get into a program? You complete medical school, then apply to match through ERAS. (google it)

Yes, you have to take the USMLE. You have to do very well on the USMLE, especially since you're foreign.

No, you don't have to complete General Surgery - you match directly into neurosurgery programs.

Yes, residents get a salary. Approximately $45,000/year starting.

Question for you: are you really from russia? If so why are you there? isn't all india institute for medical sciences good enough to get into a US program?

There are qutie a few AIMS graduates who practice neurosurgery here...

Unless of course you just randomly picked russia which is totally understandable and makes a lot of sense :laugh:
