How strict are residency class sizes? For example, if a program fills every year, do they still have the flexibility to add another resident for the usual reasons residents tend to transfer in their PGY-2+ years (ie family reasons, program dissatisfaction, etc)?

Not without getting special permission. Classes are generally not flexible at all.
