Residency to fellowship: home program vs. prestige

Are chances at competitive fellowships higher if residency was done at that same institution, or done at a more prestigious institution instead?
Specific example: if one was interested in a cardiology fellowship with Kaiser Permanente (LAMC) would it be easier to be match from a KP IM residency (LAMC) or from a more prestigious program, like UCSD IM?

Thoughts?

Depends...many of the more prestigious place dont feel the need to take their own since their residents will be competitive at other places.

Check out where their fellows come from...more in house or other places...that will give you a better idea.

Concur with the above poster about doing fellowship elsewhere. Folks often play up matching internally but there's plenty value in matching elsewhere. Having said that, CA programs are very competitive and internal applicant preferences vary greatly b/w programs anyway. Best to look at match list or fellowship rosters. Nevertheless, it would be foolish to do IM at a lesser program hoping to match in their fellowship.
 
Completely variable from place to place and I would (like mentioned above) look at trends on the websites to see how many in house they take. I would say 30-50% in house is a good range. Above or below could be a red flag. Staying in house vs going away has pro's / con's to each.
