I am curious to know what are the most competitive residency programs... I dont mean the location but rather the focus, i.e. pediatrics, surgery, family medicine, etc.

From most to least, by Step 1 scores roughly:

Plastic Surgery
Dermatology
Otolaryngology
Neurosurgery
Radiation Oncology
Diagnostic Radiology
Orthopedic Surgery
Ophthalmology

Keep in mind that while Step 1 scores are important to some extent, that's not the only factor when applying to residencies. A lot of programs look at others things like research and how good your letters of rec are. You'd be surprised how far a letter of rec from a well-known person in the field can take you.
