I am attending a MD school, but my finance is attending a DO school. Is it possible to do a couples match for residency and how would we go about it? Would we be able to do this with more competitive residencies?

I highly recommend moving this to the eras/nrmp forum. It's pretty busy these days and someone can probably help you out.

If you both apply to ACGME residencies (i.e. "MD" residencies), then yeah you could couples match. This would mean your fiance would have to take the USMLE instead/in addition to the COMLEX.

If he/she applies to DO residencies, I don't think there is a way to "cross" couples match. However, he/she would have their DO match results in hand before you had to turn in your rank list, so you could heavily weight your list based on your fiance's results.

In terms of competitive residencies - it will all come down to both of your individual strengths as candidates, but in general couples matching does make it harder. You also have to consider that (in general) the more competitive the field, the worse the DO bias.

Just wanted to point out that you posted this exact same thread in the osteopathic forum but said you were attending the DO school and your fiance was attending the MD school o_0

That is even field dependent. There are plenty of ACGME programs that will accept COMLEX only without taking USMLE (less competitive like FM, IM, or peds).

I noticed the OP must have typed his post on a cell phone because his fiance was converted in to his finance.

Unfortunately, its a TOS violation to post the same thread in multiple forums.
