Our program is going through a growth spurt, and we are trying to figure out how to handle the changes. It would be really helpful to know how other programs handle call. If you have time, please answer the following questions:

1) On inpatient rotations, how many patients is each resident responsible for at one time?
2) How often are you on call?
3) Is call in-house or home call?
4) How many patients is the on-call resident responsible for on weekends?
5) Is weekend call for the entire weekend or just for one day/night?
6) Who handles consults on weekends?
7) Do you take call throughout residency, or is call limited to PGY-2 & 3 classes?
8) How may inpatient months do you have?

At RIC:

1)On inpatient rotations, how many patients is each resident responsible for at one time?
6-19 (average probably about 15)

2)How often are you on call?
PGY2 year about 2/mo, PGY3 year about 1/mo, PGY4 yr 2 times 1st 6mo, no call thereafter

3)Is call in-house or home call?
in house

4)How many patients is the on-call resident responsible for on weekends?
on call resident always responsible for about 150 patients including pediatric patients. on call resident only responsible for floor calls - not admissions (even if late admission) - although we all help each other.

5)Is weekend call for the entire weekend or just for one day/night?
"Call" is always overnight up to 30 hrs (24hrs for Fri & Sat call), "notewriting" is all weekend admissions (btwn 4-12 per weekend split btwn two note writers) +/- responsible for up to 40pts (depends on attending what u have to do - i.e. write notes, check labs, etc.)

6)Who handles consults on weekends?
no consults just "clearing" patients for admissions by notewriters.

7)Do you take call throughout residency, or is call limited to PGY-2 & 3 classes? see above

8)How may inpatient months do you have?
12mo + 2mo (community hospital) = 14 mo of which 2mo of Peds is considered 1/2 inpt 1/2 outpt due to low volume (only 6-10pts per resident) and abundance of peds clinics - so roughly 13months. We have continuity clinic 1/2 day per week starting 2nd half of PGY2 yr so that time also gets subtracted out as well as 15 work days of vacation, 5 conference days, and 5 sick days. I think it ends up being about 12 months total.
