While I have not interviewed for the IM program, after working there several years ago I can tell you that they traditionally do not fill and often fill IM spots with IMGs (Russians seemed particularly popular in the early 90s).

Such programs do often offer positions outside of the match, but it is considered bad form to request them to do so - ie, if they like you, they may offer; you should wait for them to do so. However, you CAN ask the residents if there is a history of offering pre-match spots and express your interest in getting one of those. Some programs prefer to stick with the letter of the NRMP law and wait until scramble day to fill those positions. So don't feel bad if that's the case with UCSF-Fresno.

IMHO, if you are seriously interested in the program let them know, visit or call again after the interview (if possible); have someone call on your behalf (saying how impressed you were with the program) and provide any additional "evidence" of what a good candidate you would make.
