So are we supposed to get a response, whether interview invitation or rejection, from every program that we apply to? I've heard back from only 10% of the programs that I have applied to (IM) so far and it is already mid October (I applied on Sep 1st). Not too sure if that means most of them have already rejected me and just simply not bothered with letting me know about it or what the deal is? Many of the programs that I have applied to are not exactly high tier programs so I wouldnt think they are waiting on MSPEs to come out. Any thoughts on this matter?

Why are you nibbling around a question that has been asked and answered multiple times in multiple threads? If you bothered to do a search you would know that 1) many programs wait until the Dean's Letters are released; and 2) all programs have an incentive to retain you in a holding pattern until they are 100% sure that they have interviewed, will interview, or could possibly interview every other applicant that they would prefer to interview over you -- and if you are in a holding pattern, that definitely means you aren't getting a 'rejection' letter, and any sort of hold/waitlist/etc letter means nothing.
