What are your opinions? I do not use with pregnant women or those likely to become pregnant or most men due to its hormone altering effects. Is this a real concern for men? I am seeing mixed reviews and the iatrist I work with uses it alot.???

This just in...

I haven't been using it b/c I've been skeptical that it would be very effectiv in my mentally ill and substance dependent patient population, but I may rethink that.
 
I think like any new med, we need to be cautious with this. Several medications have effects that are only discovered after its been released to the public

I haven't used it much, but I am considering trying it in patients who fit the right profile for it..e.g. they need a med that will last through the night, or perhaps they have a circadian related sleep disorder.

Further I would consider it (though with a grain of salt) for those who may benefit from the effects of melatonin which do not involve sleep. Unfortunately many of these benefits are not on the order where standard of care suggests melatonin for use. E.g. protection from radiation, benefits in perimenopausal women, etc.
 
From my moderate experience it is not at all effective especially if your patients are used to using benzos, or ambien etc any of the like-I personally have tried it-as well as trying benzos, ambien etc and the feeling is just completley different-I can completely understand why it has no addictive qualities. My patients as well as 2 family members with chronic insomnia who were on ambien all say it is useless-it just does not "conk" them out in the same way they are used to-

actually it is mildy similar to the complaint I am sure lots of you understand in patients that switch from ambien to ambien xr-if you dont up the dose, then they do not get that same initial 10mg "conk" all at once and believe it or not that is the same feeling they get with rozerem except they also lack the sleep that follows.

So far not very imressive. However it has some utility so far that atleast I have used in much older patients with maybe some dementia etc etc who basically cannot have one of the benzos or anticholinergs/antipsychotics for one reason or another-they have some decent utility in these situations.
