Rx was given to wrong pt, is it serious mistake?

Please give some advice:

Patient returned the Rx. Pharmacist filed a report about this mistake and asked the part time intern sign it. Will this intern get some trouble? What's the consequence?
 
If it keeps on happening then yes, it is a problem but if this is a first time and the patient didn't take the med, then it's more of a warning. Be careful the next time.
 
Assuming they didn't take any...probably a slap on the wrist and a stern talking to.

If they ever want to fire you in the future, this goes into their "ammunition" folder along with every other minor mistake you will ever make. Of which there will be plenty.
 
As in intern, I don't think you have anything to worry about. Although, I do agree this can be used against you in the future by whatever co you work for. Mistakes are going to happen, but how big thos mistakes are depend on you.

What was the part-time intern's role in the mistake?
 
Did the intern actually HAND the wrong med to the patient? There is really no excuse for that, if all appropriate verifiers are used. It has happened to me once, when the pharmacist saw the patient at the window and handed me the wrong meds (she got confused between two patients who had the same last name). But in that instance, it was also my fault b/c I skipped checking our normal verifiers b/c I trusted the pharmacist. Lesson learned: always verify for YOURSELF.

My pharmacist took responsiblity for that and didn't throw me under the bus, but I could have gotten in trouble too.
 
Need more detail. This is usually a system error as pointed out by All4MyDaughter. The one thing is that it should be reported to your privacy office as HIPAA violation.
 
at my old store we had a clerk that would missell all the time. nothing happened to her. she barely spoke engrish. my complaints fell on deaf ears. depends on who you work for i reckon
 
Mistakes are as serious as the results they cause. This could have been not a big deal, and this also could have been a very serious mistake depending on the drug.

Two examples from the pharmacy I work at:

#1 - Pharmacist picked up wrong bag and handed it to the tech, tech didn't verify the name, address, DOB. Instead of getting meds for blood pressure, patient got Oxycontin. Patient took a couple of pills, then his wife read the bottle and called up the pharmacy. Nothing serious happened, just informed the patient's MD, they returned it, we move on.

#2 - Tech picked up wrong bag from the bin. The prescription bag belonged to a middle aged man, but the person was picking up for an infant by the same exact name. The bag had Digoxin 250 mcg in it. 😱

Luckily, the tech realized the mistake as soon as the patient walked away when the pharmacist asked the tech if he should mix the Amoxicillin (intended drug for infant). Tech ran out to the parking lot, and got the prescription back before it reached the patient.

They need a starting pitcher not named Oliver Perez or John Maine. And they need Beltran, Bay, Wright, and Reyes to get their act together at the same time.
You'd think a player with a $12,000,000/yr contract (cough) Oliver Perez would be able to pitch a legit game. As a bystander I'm surprised that a team with such financial backing and strong batting & bullpen still can't perform against a broken team (like Arizona). The DBacks have had such a ****ty year and STILL manage to sweep a team 2nd in their division! lawlz. epic fail.

No, being a Mets fan is the worst because they always put your through these spells when they are teasing you and then the next month they are ****ty.

2006 = That was the Met's year to win the World Series. Both Pedro Martinez and El Duque got injured in September and that left them with a tattered rotation that got them to within 1 game of the World Series. I'll never forget when Carlos Beltran struck out looking. Adam Wainwright threw a hanging curveball that Beltran could have easily deposited into the bullpen for the game winning grand slam.

2007 - Leading the division by 7 1/2 games with 3 weeks to go. Couldn't beat the lowly Washington Nationals or Florida Marlins. Coupled with the Colorado Rockies and Philadelphia Phillies resurgence during the last 3 weeks, they got eliminated on the last day when Tom Glavine gives up 7 ****ing runs in the 1st inning.

2008 - Again, they're ahead by 3 1/2 games with 3 weeks to go. Same ****ing story as 2007.

2009 - Injury plagued.

2010 - The Phillies are injury plagued this year, but the Mets can't seem to break away from them. Braves are a surprise too. And what the ****, we traded away Billy Wagner last year after his Tommy John surgery. Lo and behold, he is now the best closer in the National League. These guys seem to get the Tommy John surgery and come back throwing harder than before.

Consider yourself pitied...although my team took your gold glove center fielder in McClouth and now he cant even hit above .200...FFS.

And Sparda, yes the Braves have been doing much better than anticipated, and I dont hate it. Send Bobby and Chipper out with a bang 😀

Ah. Yeah, people don't seem to understand that the best member of the Pirates doesn't mean much. They get to play the Astros and Brewers all year..multiple times. Me and 8 quadriplegics could beat the Pirates.

The time they shot off the fireworks for Lastings Milledge hitting a double was classic. Yes, folks, they're that bad. They celebrate when a player hits a double.

The Pirates have sucked since before I was born. But at least bringing them up is a convenient way to shut up Yinzers when they start gabbing on about the Steelers. Yes, we get it, six Super Bowls, but at least my football team's quarterback isn't a pervert and an idiot. Oh, and the Penguins. We get it, Sidney Crosby was dubbed as the hockey equivalent of Jesus at the beginning of his career, and now the media falls over itself trying to find any highlight that might validate their claim, trumpeting common achievements as being extraordinary simply because Crosby performed them.

That's why I don't feel that much sorry for Pittsburgh, they have a decent football team and a decent hockey team.

Alright, I'm here in NYC and we have the Yankees but don't forget, for a Mets fans, the Yankees are a ****ing constant reminder of what the Mets are not. And yes I'll admit it, deep down, all Mets fans want the Mets to be like the Yankees.

The problem with the Mets is not the players, its not the manager, its not the general manager. The problem is the ****ing ownership who are not willing to "win at all costs". I wish George Steinbrenner had bought the Mets instead of the Yankees.

Former Pirates outfielders currently playing crappy on other teams: Jason Bay, Nate McLouth, Xavier Nady. Nyjer Morgan, oddly enough, is doing so-so.

Jason Bay was playing great with the Boston Red Sox. The former pirate that has probably been the cause of the Mets not making it further from 2006-2008 = Oliver Perez.
 
and I played little league, Babe Ruth... middle school...HS baseball...

It was fun playing...absolutely no enjoyment in watching it.

I tune in for an inning or two now and then.

Yesterday I witnessed the Pirates walk in a run with the bases loaded. Then about 1 minute later, they put the tarp over the field and it wasn't even raining. As if the ground crew chief couldn't handle it anymore and just decided to make them stop playing.

Watching them fail miserably is fun.
