Scheduling M4/residency app?

So...COVID kinda sucks. Destroyed lives and it really messed up the plans I had for step studying/scheduling my rotations.

As of right now I am applying to internal medicine. I have a departmental letter of rec from my internal med rotation, + an attending from the floor, and from my FM site. I3 so far, it's just one is from FM and not internal.

I'm currently finishing up M3 with a pretty chill rotation, but the board is in 2 weeks and I'm scheduled to take step 2 in 5 weeks.

I have 3 weeks after my board exam to study for step 2. Although I have been studying this entire time, my goal is to attend a university affiliated residency program so I can apply for a competitive fellowship. 3 weeks is a bit crunched, and I would prefer to use 4/4.5 weeks instead to truly feel prepared. I did average on step 1, and i wanted a much better step 2 score to seal the deal. However, the issue is that I have a Cardio elective which begins August 31st.

I don't think I would study well while in a rotation like cardio, so I am wondering if I should reschedule my cardio rotation for later in the year?

I was hoping to get a letter of recommendation from this cardio rotation (to replace the FM letter), but I am wondering if the 3 letters I have now will suffice. I also plan on getting a letter from my ICU rotation which is right after the cardio--it would go in 2 weeks after ERAS opens--but shouldn't be too much of an issue?

I'm wondering if it's a really big deal to have an FM letter instead of one from an internal person?
 
Unless you want MGH IM, I think you're unnecessarily stressing. If you're USMD with no red flags and avg step 1 you should be gucci (meaning delaying cardio and using your FM letter or the other way around will work)
