From what I've seen on SDN, I would just put down anything from college and beyond. I assume you are asking this question because you got Eagle Scout during college? If so, I think it might be worth it to put it on the app.

Nah. They don't want to hear about your high school career. I wish they did, a lot of us have some good things we could have added, but they're really only interested in what you've been doing through college. I don't know how the Boy Scouts work, but if you were still active beyond HS, then I would assume it'd be okay.

No I got it when I was in high school. I was told it would look impressive on any application

Keep in mind a few schools like CCOM ask for a CV. This may be an appropriate place to indicate things like that.
