Three things happened:

(1) The difficulty of the practice exams are not comparable. I remember getting a 264 on one for Step 1 and was all like (YEAH, GONNA RAPE THIS ****) and then the next day got like a 220 (awh... i should just quit). So, chances are, its not you, just the exam. Study More

(2) You weren't in the mindset. If you were totally focused, rely on number one. If you weren't its not surprising. You aren't going to have a good feel for the test until you sit down and are forced to be on game day. Though you should definitely prepare as close to the real deal as possible. Study More

(3) Worst case: you are past your prime. I fundamentally believe that studying for too long is detrimental. I think that happened to me on Step 1. I studied for 8 weeks. The last two weeks was just Uworld, every day. I think I missed my prime (at 6 weeks) and ended up doing poorer because I was burned out. If that's the case, its either time to take a break or just take teh test. Forcing yourself to study more will cause you to fail... and end up like everyone who had a retirement fund in the stock market. Stop Studying and take the test!
