I have a manuscript about to be submitted for publication. The author listing goes like name1†, name2†, myname3‡, name4§, name5§, PI* . The first two listed are co-first authors. I am listed third. Am I second author or third author? Not sure it makes much of a difference though.

I'm second author on a paper, and my mentor has submitted it to one journal already. We were turned down so we're working on revisions before re-submitting to a different journal.

I know it takes about a year or more before some stuff actually gets published. Would I have to wait for our article to be accepted and/or published to list it as a publication in an update letter to med schools?

I would hold off on updating them because you certainly can't list it as an actually publication yet. If you have already told them (in the primary or secondary) about the submission, then to update them now only shows that it got rejected somewhere. If you haven't yet told them about the submission, I think it would be appropriate to update them when:
a) you submit it
b) the journal asks for revisions and a resubmission to the same journal (tell schools that your paper is currently "in revision" at whatever journal)
c) it gets accepted for realsies

You don't have to (and probably shouldn't) update for each step, it mostly depends on your timeframe for the paper.

if you have other stuff to update them on and was already planning on sending an update letter you should include that you submitted a manuscript to x journal

...on the other hand it's already january and unless you are updating a school that has already interviewed you or invited you for an interview it's probably not going to make a difference

except a lot of schools are still inviting for interviews..
 
It's for a school that I've already interviewed at. But they don't accept update letters until February anyway, so I'll wait til then to see how the timing of the manuscript all pans out. Thanks!
 
If you have a competed manuscript being evaluated for publication, you can definitely include it on your CV, with the expected citation listed as under review or something to that effect. Of course it will be missing the ultimate journal, but it's totally appropriate to include in your written materials.
