Secondaries information for applicants of 2006 cycle?

I think this wold be good idea to compile the secondaries information of all schools for all the applicants of 2006. Which Schools require just Money and Picture as a Secondaries? Which Schools mail the secondaries and which ones have online secondaries that you need to take initiative to fill out? Please post here with school name and secondary information.

Hey guys

A lot of schools listed (the ones that want you to Send other materials to the dental school at the same time you send AADSAS materials) on that website want you to submit your DAT scores as well as paying the sec. fees. I haven't taken the DAT yet. Would it be late if I submitted the secondaries (fee/ supplemental application) after taking my DAT (in a month and half)?
